The Cobalt Rehabilitation Hospital of New Orleans is a destination hospital dedicated to serving to a wide range of physical rehabilitation patients such as stroke, trauma, hip fracture, brain injuries, concussions, orthopedic, Parkinson’s disease as well as many other inpatient and outpatient diagnoses. Each patient stay includes upscale, hotel-like amenities.

In addition to the Brain injury and concussion program, the facility also features a Parkinson’s program, 60 private inpatient suites, as well as both inpatient and outpatient programs.

Scott Tranchina

Scott Tranchina is the Chief Executive Officer of Cobalt’s New Orleans Hospital. Prior to joining Cobalt, Mr. Tranchina served as the CCO and promoted to CEO, of Kindred Healthcare’s LTACH Hospital in New Orleans. He has over 18 years’ experience in healthcare in various positions, as CEO of post-acute facilities for Inpatient Rehabilitation and Long Term Acute Care, Chief Clinical Officer, Director of Inpatient Rehab, Director of a primary certified Stroke program, Director of Orthopedics, Cardiac Rehab specialist.

Mr. Tranchina has developed his career in healthcare management with an expertise in leadership staff development, healthcare operations, and hospital finance. Specializing in program stabilization and process development. He is a certified brain injury specialist and recipient of the Great 100 nurses award for Louisiana.

Mr. Tranchina was born and raised in New Orleans, La, married with four children. He earned his Bachelor’s degree in Kinesiology from University of New Orleans. Mr. Tranchina played professional baseball in the Chicago Cubs Organization prior to returning to earn a nursing degree from Charity School of Nursing, and later a Master’s degree in Healthcare Systems Management from Loyola of New Orleans.

Gwen Faucheaux

Mrs. Faucheaux is a professional registered nurse having completed a nursing diploma program at Charity Hospital School of Nursing in New Orleans, LA. Continuing her education was important to her leading to obtain a Bachelor’s Degree in Health Arts from the College of St. Francis in Joliet, IL and finally a Master’s of Science Degree in Healthcare Management from University of New Orleans in New Orleans, LA.

Working in management to lead all hospital staff to provide safe care professionally in a safe environment has been Mrs. Faucheaux’s life work. She has held positions such as Staff Nurse, Intravenous Therapy Nurse, Administrative Representative, Associate Director of Nursing, Infection Control Practitioner, Employee Health Nurse, Staff Educator, and Chief Quality Officer.

During her working career, she has successful lead several hospitals through Joint Commission Surveys, Health Inspections, Fire Marshall surveys, and been given several Golden Attitude (employee recognition) and Manager of the Month awards.

Outside of the hospital environment she enjoys spending time with her six grandchildren. Giving them love, understanding and leading them to be productive, and honest has given her boundless joy. The love of travel especially cruising to unfamiliar places and discovering the history and culture of native lands fills her leisure time.

Professionally, Mrs. Faucheaux has belonged to professional organizations such as the Association of Infection Control Practitioners (APIC) where she served as Regional Membership Chairman, local group Treasurer, President Elect, and President. Civically she has been a member of the Republican Women’s Committee when Louisiana was moving from a democratic to a republican state and still remains active.

Angelle Osborne

Angelle Osborne is the Human Resources Director for Cobalt Rehabilitation Hospital of New Orleans. Offering over 20 years’ experience in various operational and HR roles, she oversees all aspects of Human Resources for the staff at the facility including recruiting/talent acquisition, compensation and benefits, employee development, and overall support staff for the facility.

Angelle joined the Cobalt family in May 2016. She brings passion and enthusiasm in her role as Human Resources Director and helps uphold Cobalt’s core values of teamwork, integrity, respect, fun, professionalism, and responsibility every day. Angelle strives to be a resource and support the staff of Cobalt in any way she can and seeks ways to make the employee work environment exceptional. She enjoys working with her counterparts ensuring that human resources technologies are aligned and consistent across the entire Cobalt enterprise.

Prior to joining Cobalt leadership team, Angelle served as the Director of Human Resources and Accounting Director for Earthsavers for 7 years. She was a key player in the expansion of the Earthsavers brand with the addition of Well. Carrying many of the responsibilities in order to launch and open.

As New Orleans native, Angelle enjoys music and southern cuisine and any free time is devoted to her family.
